What Youll Do in This Role:
As a Community Care RN with our PACE program youll be at the heart of a missiondriven team providing both direct and indirect care to older adults in our community. While youll work closely with the Nurse Manager or a designee for guidance your advanced clinical skills and judgment will shine as you navigate and manage complex health situations with confidence and compassion.
Primary Responsibilities:
Assess & Plan Care
- Complete thorough assessments and home visits for new and current participants.
- Identify care needs set priorities and develop individualized care plans.
- Collaborate with Physical Therapy and act as a backup Triage Nurse as needed.
- Participate in team meetings and coordinate care with the full IDT.
Deliver & Manage Care
- Provide treatments like wound care IV therapy and tube feeding.
- Manage medicationsfrom enrollment through education and pharmacy coordination.
- Monitor participant health respond to emergencies and document care accurately.
- Support transitions of care including hospital admissions and discharges.
Lead with Purpose
- Attend daily huddles and support smooth clinic operations.
- Delegate tasks appropriately and mentor team members.
- Promote safety participate in quality improvement and champion best practices.
What Youll Need:
Required:
- Graduate of an accredited School of Nursing
- Active RN license in the State of Michigan
- CPR certification or willingness to obtain
- Valid Michigan drivers license and access to reliable transportation
Preferred:
- 3 years of nursing experience in a healthrelated setting
- At least 1 year of experience working with older adults or a frail population
