Job Title: Physical Therapist (PT)
Job Location: Our Physical Therapists see patients in the most ideal setting the patients home!
Our offices are located in Dunmore PA and East Stroudsburg PA. At Traditional we do our best to keep each PTs case load within a sensible geographic territory.
Shift(s):Fulltime positions available.
Duties and Responsibilities include but not limited to the following:
- Evaluate patients and develop plans of care to achieve desired outcomes
- Provide therapeutic treatment modalities and implement home therapy / exercise programs
- Provide instruction to patients to achieve patient goals
- Maintain communication with ordering physicians and clinical team to promote progress toward goals or seek intervention when needed
- Participate in quality improvement activities as assigned
- Timely and accurately document (EMR) all patient assessments interactions etc.
Requirements:
- Must possess a current valid Pennsylvania PT license and have current CPR certification
- Valid drivers license and ability to travel to case assignments
- Must be comfortable working in home setting
- Ability to work independently with minimal supervision and manage time effectively
- Strong interpersonal and communication skills
