Description: This is responsible and specialized case management work in assessing planning linking monitoring and advocating for a caseload of adults with serious and persistent mental illness. The incumbent is responsible for assisting clients with their needs/goals through the development of the treatment/service plan and/or aftercare plan. Work also entails responsibility for working with their families or guardians community resources and advocating for clients by linking them with appropriate services.
Job Duties: - Provide case management/therapeutic mentoring services to an assigned group of clients
- Meet with assigned clients on a weekly basis or as indicated by treatment planning
- Act as a liaison and consult with school systems community agencies and families to maintain coordination of the treatment process
- Advocate for families rights and needs
- Refer adults and families for additional services as identified
- Meet expectations regarding documentation deadlines and utilization trends
- Preparation of social skill building activities for each client
- Assist with assessing safety issues that may have an impact on the service
- Provide transportation respite and tutoring services as assigned
Requirements: - Bachelors degree in human services or related field
- Six 6 months experience required one 1 year of experience preferred
- Valid drivers license with a clean driving record
