Job Description

Reporting directly to the Director Community Integration & Primary Health Care the Project Manager will be responsible for the coordination and management of a wide range of projects supporting the advancement of integrated communitybased care initiatives.
Key areas of responsibility include the implementation and oversight of initiatives such as SCOPE (Seamless Care Optimizing the Patient Experience) Remote Care Management (RCM) Central Intake & Coordinated Access Outpatient Rehabilitation services Social Work/Discharge Planning and the advancement of Primary Health Care initiatives in collaboration with the Connected Care Halton Ontario Health Team (CCHOHT).
This work will span across the Oakville Milton and Halton Hills communities and throughout the broader Central Region.

Key Responsibilities:

  • Lead and manage a diverse portfolio of complex projects aligned to the Community Integration & Primary Health Care strategy and broader Halton Healthcare corporate goals.
  • Provide leadership in developing and strengthening partnerships with primary care providers and community health organizations collaboration initiatives.
  • Collaborate with internal and external stakeholders to conduct needs assessments system/process redesign and project planning activities; provide recommendations that improve service delivery and enhance integrated patient care models.
  • Provide project leadership on multistakeholder initiatives with a focus on innovation community collaboration and improving transitions of care across hospital primary care and community settings.
  • Conduct research and apply best practices related to community health primary care integration and hospitaltohome transition programs; propose solutions that are sustainable costeffective and patientcentered.
  • Monitor track and report on project plans deliverables risks and mitigation strategies to leadership and stakeholders.
  • Liaise with internal hospital departments primary care partners community agencies consultants vendors and government stakeholders to ensure that project objectives and timelines are achieved.
  • Exercise a high degree of autonomy in planning prioritizing and managing projects ensuring alignment with strategic priorities.

  • Provide strategic and operational support to program leadership to advance new initiatives and strengthen partnerships aligning with shared initiatives 

  • Contribute to broader Community Integration & Primary Health Care and corporate initiatives committees and working groups as needed.


Qualifications :

  • Bachelors Degree in Health Administration Health Sciences Business Administration or a related field; Masters Degree preferred.
  • Project Management Professional (PMP) certification considered an asset.
  • Minimum 5 years experience managing complex health system projects ideally in hospital community health or primary care settings.
  • Knowledge of health system integration primary care structures community care models and Ontario Health Team (OHT) frameworks.
  • Demonstrated ability to manage multiple concurrent projects with competing priorities.
  • Strong leadership collaboration communication and problemsolving skills.
  • Ability to work independently and collaboratively across multidisciplinary teams.
  • Proficient with project management software Microsoft Office Suite and reporting tools.
  • Flexible and enjoy working in a fast paced environment
  • Liaise with a broad range of individuals from clinical and professional staff to external stakeholders
  • Research skills of best practices for a wide variety of areas related to complex projects. Verbal written communication and presentation skills essential


Additional Information :

Hours: Monday to Friday 7.5 hour day shifts (Hours are subject to change based on operational requirements)

Site: MultiSite primarily Oakville

Salary: $53.083 to $65.767 per hour
