The Position: The Nurse Practitioner occupying this role with Snowline ElderCare will deliver comprehensive personcentered primary and palliative care services to seniors in homebased clinical settings. This position emphasizes quality of life health promotion and chronic disease management aligning care delivery with the mission and values of Snowline Health as well as the local community. The provider will work collaboratively with the multidisciplinary team to address complex health needs coordinate care and ensure patient and family satisfaction. The providers in this role will play a key role in care plan development and optimizing transitions of care for program enrollees.

Responsibilities: As a Nurse Practitioner (NP) at Snowline Health you will be responsible for providing compassionate and comprehensive nursing care to our patients and their families. The position is based out of Diamond Springs CA and involves a hybrid of onsite and remote work. The role focuses on providing comprehensive patientcentered primary and palliative care to elderly patients in homebased and clinic settings.
Your duties will include:
Clinical Care providers in this role will confidently be able to deliver in residence care and develop comprehensive care plans for complex patients with team coordination.
Conduct comprehensive health assessments
Develop and manage individualized treatment plans
Prescribe medications and treatments
Provide patient and family education
Perform necessary procedures within the scope of practice
Document patient encounters accurately and timely in EMR
Participate in call coverage rotation
Care Coordination
Collaborate with multidisciplinary team members
Serve as the primary point of contact for patients and families
Coordinate referrals to specialists and community resources
Program Development
Assist in developing and refining clinical workflows and protocols
Participate in quality improvement initiatives
Provide mentorship and training to clinical support staff
Regulatory Compliance and Quality Assurance
Ensure compliance with state federal and organizational regulations
Participate in internal audits and reviews
Maintain current knowledge of geriatric and palliative care practices
Familiarity with CMS billing and coding for homebased care
Maintain all required professional credentials and continuing education
Community Leadership and Engagement
Represent Snowline ElderCare in community forums and events
Act as a resource for staff and community members on elder care best practices
Demonstrate delivery of highquality compassionate homebased care
Mentor students and other team roles as appropriate
HIPAA Minimum Necessary Access
Access to clinical client health information
Current valid California license
Minimum of three years of clinical experience in geriatric palliative or primary care
Proficient in medical terminology EMRs and HIPAA compliance
Knowledge of palliative care and hospice preferred
Experience in homebased care delivery preferred
Strong clinical assessment and diagnostic skills
Excellent interpersonal communication and organizational skills
Ability to work independently and collaboratively as part of an interdisciplinary team
Valid CA drivers license with a reliable insured vehicle for home visits
Active and updated DEA/CURES credentials
Knowledge of traumainformed care principles preferred
