Job purpose
Reporting to the Program Manager for Cancer Care Health Equity the Community Health Worker for Cancer Care (Lung Cancer Screening) is a community ambassador educator and navigator for cancer prevention at Fair Haven Community Health Care (FHCHC). The role coordinates FHCHCs patientfacing efforts at preventing cancers of all types including breast cervical colorectal and lung. The role effectively communicates about cancer prevention in ways that resonate with communities served by FHCHC.

Qualifications
Associates degree relevant experience and demonstrated knowledge of communities in New Haven is required. A Bachelors degree preferred. Bilingual English and Spanish and proficiency with Microsoft office is required. Community Health Worker certification and experience in health care with electronic medical records systems are highly desirable.
