Description


THE POSITION: Under supervision and as a member of our Ambulatory Care Enhanced Care Management Team the fieldbased Licensed Vocational Nurse (LVN) will assist with providing skilled nursing care within the scope of vocational nursing practice in the State of California.

The fieldbased LVN plays an integral role in coordinating services for patients with complex needs. Offering compassionate case management and support to help them achieve their health goals. In this role incumbents will facilitate care planning goal setting and motivational interviews guiding patients through medical behavioral health and social services. Additionally the fieldbased LVN will conduct fieldbased care coordinate with providers and efficiently document case notes and services provided in the electronic health record system. The Per Diem Primary Care Clinic Licensed Vocational Nurse is distinguished from the Primary Care Clinic Licensed Vocational Nurse series in that Per Diem Nurses are scheduled to work on an asneeded basis (Intermittent) to supplement staffing needs.

IDEAL CANDIDATE: The ideal candidate will understand Enhanced Care Management (ECM) services and their importance in assisting patients with complex intricate and challenging health conditions. Additionally compassion and a demonstrated history of efficiently overseeing and monitoring vital health data scheduling various medical appointments arranging transportation and maintaining thorough documentation are essential qualities sought in the ideal candidate.

WHAT WE OFFER:

The County of Ventura offers an attractive compensation and benefits package subject to eligibility requirements that includes but not limited to:

Primary Care Clinic Licensed Vocational Nurse I/II:

  • Educational Incentive: Incumbents may be eligible for an educational incentive of 2.5 3.5 or 5 based on completion of an Associates Bachelors or Masters degree.
  • Bilingual Incentive: Incumbents may be eligible for bilingual incentive depending upon operational need and certification of skill $0.69 per hour (Level I) $1.00 per hour (Level II) or $1.32 per hour (Level III).
  • Certification Pay:$0.813 per hour based on scheduled work week hours for each qualified certification up to a maximum of five 5 certifications.
  • Continuous County Service Incentive Pay: 1 for 7 years 1.25 for 12 years and 1.5 for 17 years
  • Flexible Credit Allowance: You are afforded a biweekly flexible credit allowance for purchasing medical dental and/or vision insurance from authorized plans.
  • Deferred Compensation:Eligible to participate in the Countys 401(K) Shared Savings Plan and/or the Section 457 Plan. If you participate in the County sponsored 401(K) plan the County will match a part of your contribution up to 2 of your salary on a per pay period basis.
  • Pension Plan: Participate in the Countys defined benefit pension plan that provides lifetime payments to retirees. Members are vested after attaining five years of retirement service credit. If eligible you may establish reciprocity with other public retirement systems such as CalPERS.
  • Holidays: 12 paid days per year which includes a scheduled floating holiday.

Per Diem Primary Care Clinic Licensed Vocational Nurse:

Examples Of Duties

Duties may include but are not limited to the following:

In addition to the duties listed above the Primary Care Clinic Licensed Vocational Nurse II also:
  • Provides clinical support to assigned clinical work area;
  • May assist in interviewing new clinical support staff;
  • Supports quality improvement efforts;
  • Properly handles vaccine and medication ordering and storage;
  • Assists with training on workflows and duties; and
  • Performs other related duties as assigned.

In addition to the duties listed above the Per Diem Clinc Licensed Vocational Nurse level may include:
  • May coordinate clinical support staff coverage with management;
  • May lead care tema huddles;
  • May ensure clinical support staff is up to date with necessary training and competencies; and
  • Perofrms other related duties as assigned

EDUCATION TRAINING and EXPERIENCE:
Primary Care Clinic Licensed Vocational Nurse I


Advancement to the Primary Care Clinic Licensed Vocational Nurse II will be based on the meeting the criteria above and at the initiation of Ambulatory Care Nursing Administration.
Primary Care Clinic Licensed Vocational Nurse II & Per Diem Clinic Licensed Vocational Nurse
  • Requires one 1 year of fulltime licensed vocational nursing experience in a health care setting and completion of the Primary Care Clinic Licensed Vocational Nurse I competencies established by the Ambulatory Care Nursing Administrator.

NECESSARY SPECIAL REQUIREMENT:
Primary Care Clinic Licensed Vocational Nurse I and II
  • Must possess and maintain a current valid license as aVocational Nurse issued by the State of California.
  • Must possess a current valid California driver license by the time of hire and maintain it thereafter.
  • Must successfully complete the pharmacology examination within three 3 months of employment.
  • May require working in various departments and/or a variety of shifts.

DESIRED:
  • Case management experience
  • Experience working with individuals facing substance use challenges and/or those dealing with mental health issues
  • CLIA waived testing annual competency

KNOWLEDGE SKILLS and ABILITIES:
Working to thorough knowledge of:
  • the techniques and practices of nursing;
  • medical terminology routines and equipment;
  • the role in abuse/neglect reporting;
  • confidentiality policies;
  • case management; and
  • safe driving practices.

Skill in:
  • the transcription of physicians orders including telephone/verbal orders and the completion of those orders; and
  • performing CLIA waivered testing.

Demonstrated ability to:
  • apply a variety of techniques methods and procedures utilizing required equipment for patient care; and
  • cultural sensitivity and competence with patients.

Ability to:
  • administer medications as ordered by physicians;
  • follow agency and unit policies and instructions as given;
  • establish and maintain the confidence of patients;
  • maintain confidentiality of patient/client information;
  • work effectively in different healthcare and/or hospital departments or shifts as required;
  • establish and maintain effective interpersonal relations with other staff members patients and their families;
  • communicate effectively both orally and in writing; and
  • work with a high volume of patients internal/external customers and appropriately adapt to frequent changes delay or unexpected events.
