Adult Detention Center Forensic Release Planner
The Forensic Release Planner (Behavioral Health Specialist II) is a clinician on a multidisciplinary team where coworkers are therapist/case managers medical staff peer support specialists and criminal justice system partners. Serving as a primary therapist/case manager provides a wide range of professional treatment services to diagnostically culturally and socioeconomically diverse client populations with serious mental illness substance use emotional and cooccurring disorders as well as developmental disabilities.
This position will screen and access the psychiatric medical social services employment and residential needs as well as risk factors of individuals with serious mental illness at the Fairfax County Adult Detention Center (ADC). Will coordinate care with community providers and community supervision agencies including the exchange of treatment records communication of treatment need and linking the individual with available services and supports upon release from the ADC. Works with the individual for at least 30 days postrelease from the ADC to ensure a smooth transition to the community and connection to services. Works with individuals incarcerated at the ADC who have been transferred to a State Hospital and in the community after being released from the ADC. This position will require work in the Fairfax County Adult Detention Center and the community. Community work will include driving clients to different appointments and housing locations throughout the county.
The positions work hours may include weekend and evening hours as well as some holidays. This position is 100 onsite.

MINIMUM QUALIFICATIONS:
Graduation from an accredited college or university with a bachelors degree in psychology social work nursing counseling or a related field plus 1500 hours of clinical experience;
OR a bachelors degree and a valid Qualified Mental Health Professional (QMHP) certification or Certified Substance Abuse Counselor (CSAC) certification plus 1500 hours of clinical experience;
OR a masters degree in psychology social work nursing counseling or a related field plus 500 hours of clinical experience.
