About the facility
With 2 floors 4 units we are able to provide care for long term care residents and postacute rehab patients. The first floor has 96 beds and the second floor has 100 beds. Nursepatient ratios vary depending on the shift and the population you will be serving. We care for dementia residents long term care and a rehab residents. With 196 beds we accept but not limited to patient specialties such as general rehab neuro rehab cardiac respiratory and cancer. We also have various programs for residents to participate in
multiple activities.
The Social Worker provides and/or coordinates medically related social services to attain and maintain the highest practicable physical mental and psychosocial well being of each resident. This activity is achieved through the use of appropriate assessment tools development completion and revision of resident care plans and maintaining contact with residents and their family members. The Social Worker ensures that all charted progress notes are informative and descriptive of the services provided including the residents response to the service.
