Job Description

Job Duties:

  • Directly supervises Hab. Aides and individuals through regular visits and audits to assure that the 6100 regulations and MCARs Policies and Procedures are always maintained
  • Ensure the welfare care safety and security of all consumers on their assigned caseload at all times
  • Serves as a coordinator of Hab. Aid services for up to 40 individuals with intellectual disabilities residing in community situations
  • Works closely with County SC and Hab. Aid Manager to assure that appropriate services are provided for individuals assigned to their caseload
  • Acts as a coordinator for individuals to develop and assist them with obtaining any service to meet their personal medical financial social and community integration needs.
  • Initiative and independent judgment are exercised within a broad framework of rules regulations and policies
  • Prepares reports for Administrator of Habilitation Aide as requested
  • Completes and updates support plans outlining techniques to assist with developing new skills
  • Provide transportation to individuals for employment needs as necessary
  • Daily and monthly documentation of case notes on online system required

Work Schedule:

  • FullTime
  • Flexible hours to facilitate regular visitation outside of the 8:30 AM 4:30 PM business day
  • Ability to work in various environments

Job Qualifications:

  • Bachelors Degree in Education Human Service Social Work or related field preferred Associates Degree or 60 college level credit hours and four years experience working directly with persons with ID or two 2 years experience working with individuals with MH/ID disabilities in a Human Service Agency or one 1 year supervisory work experience preferred
  • Must be 18 years of age

  • PreEmployment Criminal Background Physical Examination TB Test Drug Screen
