The Opportunity:

This is an internal recruitment open current regular probationary/trial service and limited duration employees or those in a paid or unpaid internship program at the time of recruitment who are in paid status on approved leave or on an active recall list.

Temporary and oncall employees in Local 88 may apply for an internal recruitment if they were hired through a competitive civil service process.

Temporary and oncall employees who are not in Local 88 may apply for an internal recruitment if all of the following conditions are met:

  • The employee was hired into the current temporary or oncall position via a competitive recruitment and selection process consistent with the countys Personnel Rules;

  • The employee has worked in an oncall and/or temporary capacity at least five hundred 500 hours within twelve 12 months preceding the date of application;

  • The recruitment is for a position in their current department; and

  • The recruitment will not result in a promotion to a job class with a higher maximum salary rate.

If so the Aging Disability and Veteran Services Division (ADVSD) is excited to announce that our Adult Protective Services (APS) program is currently seeking an APS Risk Case Manager (Case Manager Senior) and we welcome you to apply!

The APS Risk Case Manager provides shortterm case management for vulnerable adults who are at high risk for abuse or selfneglect or have a significant threat to their health and safety. Risk case management services stabilize individuals providing intensive intervention for up to one year by creating connections to appropriate agencies and ongoing services.

Primary responsibilities include :

  • Provide specialized shortterm intensive care coordination and case management for individuals who have complex conditions or circumstances and who may need access to health and social service resources across multiple systems

  • Ensure appropriate referrals are made to other County programs or community resources

  • Report alleged abuse neglect or self neglect

  • Conduct in person assessments to determine client service needs

  • Consult with medical or other professionals as needed; reviewing interpreting and collecting medical and social information from various sources

  • Work with clients and their support networks establish a plan for services determine best methods to implement the plan and provide support and resources needed to implement the plan and access benefits

  • Provide outreach to county residents who are experiencing home foreclosure by Multnomah County DART (Division of Assessment Recording and Taxation)

  • Initiate immediate health and safety response for clients whose health or safety may be at risk

  • Assess and respond to potential safety concerns

  • Complete proper and necessary documentation in CAM (Centralized Abuse Management)

  • Other duties as assigned

The eligible list established from this recruitment may be used to fill future fulltime parttime regular temporary limited duration and oncall positions.

Type of Position: This AFSCME Local 88 unionrepresented fulltime position is eligible for overtime.

Schedule: Core business hours are Monday Friday 8:00 AM 5:00 PM. An alternative work schedule is available for this position.

Location: This position is designated as routine telework meaning you may work remotely (from home); a hybrid or in office schedule is also available. Employees are required to come into the office on occasion and regular field visits throughout Multnomah County are also a requirement. The designation of Routine Telework may be subject to change at a future time. All employees must reside in Oregon or Washington; the county cannot support ongoing telework from other locations. The onsite work location for this position is 4610 SE Belmont St Portland OR 97215.

Everyday Multnomah County staff work together to serve as a safety net for our communities. During a disaster this safety net becomes even more critical. All County employees have a role in serving the public during inclement weather natural disaster or other types of community emergency response. During these emergency responses while typically there begins with a call for volunteers county employees may ultimately be reassigned from their current position to a role in the emergency response in order to support the critical needs presented by our communities.
