The Geisinger Skilled Nursing Program is seeking an experienced Nurse Practitioner or Physician Assistant to rotate between Mountain Top Rehabilitation & Healthcare Center Mountain Top Birchwood Rehabilitation and Healthcare Center and Lakewood Rehabilitation & Healthcare Center Nanticoke PA.Job Duties:
This position is as a Geisinger provider in a contracted skilled nursing facility and requires an experienced advanced practitioner prior experience working in a skilled nursing facility family medicine internal medicine or ICU. The advanced practitioner (AP) provides direct (handson) care in skilled nursing facilities with a goal to improve quality and prevent unnecessary readmissions to the acute care setting.

Responsibilities:
- Review reports
- Communicate with SNF staff for patient needs
- Treat patients for acute needs admissions discharges etc.
- Manage extremely complex patients see every day or at least once a week
- Approximately 12 patients a day
- Length of time per patient and number of patients is per need
