Job Description
Hope For Youths Intensive & Supportive Case Management (ISCM) program is an outpatient preventative service that provides children and their families with the support and treatment needed to stabilize a youths immediate living situation and prevent the placement of children into foster care or a higher level of care. The ISCMs intensive track (ICM) can accommodate the needs of up to 12 children and their families. The ISCM Program is a contract program through the Suffolk County Department of Social Services and services families throughout Suffolk County.
The ICM track is designed to help prevent disruption multiple placements and placements in more restrictive settings. Hope For Youths caseworkers utilize proven screening tools such as the Casey Life Skills Assessment and Child and Adolescent Strengths and Needs Screening. ICM staff work closely with the DSS. Through interdisciplinary assessments DSS and Child Protective Services provide the Hope For Youth ICM team with information pertaining to the families past and current functioning so that Hope For Youths Intensive Case Managers can provide recommendations for going forward. Communication between Suffolk County and HFY is ongoing.
To be a successful Intensive Case Manager you must be ready willing and able to do the following always:
- Provide services to maintain child in current living environment and work to achieve positive outcomes/deliverables as outlined in the service contract/grant.
- Assess and address safety and health concerns in the childs current home environment and provide appropriate advocacy as needed.
- Conduct twice weekly face to face contacts/home/community contacts and completes all necessary reports paperwork and forms as outlined in the program operations manual.
- Maintain a caseload up to 4 clients.
- Perform other resulting and necessary work to meet the needs and requirements of the clients the program Hope For Youth oversight agency and any other regulatory obligations
How will you generally spend your time in this job:

- Other assignments as needed
To qualify for this job you need to be or have a minimum:
- BSW or bachelors degree in a related field
- Valid NYS Drivers License in good standing
- Access to a reliable vehicle that can be used on a regular basis for home visits and transports throughout Suffolk County.
- Excellent writing skills/time management.
- Ability to be flexible with scheduling (work evenings possible weekends if necessary).
- Responding to all calls/emergencies via agencyprovided mobile phone and/or via internet technologies
- Physical Requirements: read/write reports and log entries walking/standing/sitting for prolonged periods bending/stooping kneeling/squatting climbing stairs reaching overhead repetitive wrist/finger movements ongoing access to reliable vehicle and driving own vehicle for work
This position is being offered as Full Time Monday Friday 9a5p: flexible
