Utilization Review Discharge Planner

Jefferson Healthcare is currently seeking a parttime Utilization Review RN to join our Case Management Team. In this role you will be responsible for planning organizing coordinating and conducting reviews related to risk quality compliance and utilization management. Additionally the UR Discharge Planner is responsible for all aspects of discharge planning and ensures the timely and seamless clinical transition of patients through medically appropriate levels of care. This position requires meeting with hospitalized patients and their families to develop an appropriate discharge plan.

The position requires the ability to handle confidential and sensitive information concerning the performance of primary care providers nurses ancillary and support staff in delivering medically appropriate care. You will be expected to develop and maintain a system for the utilization review of incoming patients as well as implement audits for concurrent reviews of utilization quality and the related performance of patient care staff. Additionally you will perform professional activities related to Risk Quality Compliance Management and Utilization Management (QM/UM).

What youll need:

  • Current Washington State RN license in good standing
  • Current AHA BLS/CPR (for Healthcare Providers) required
  • Two years of experience in a hospital/clinical environment preferred
  • Bachelor of Science in Nursing preferred
  • Previous leadership experience preferred

Schedule: 0.5 FTE; 20 hours/week; Variable Shift
