Job Description
Senior Physical Therapist Assistant Home Health Care Specialist
Join our growing healthcare team as a Senior Physical Therapist Assistant (PTA) specializing in home health care. Were seeking a compassionate and skilled professional to deliver exceptional therapeutic care to patients in the comfort of their homes helping them achieve optimal functional independence and quality of life.
Essential Responsibilities:
- Implement personalized treatment plans for orthopedic and neurological patients
- Provide expert therapeutic interventions including manual therapy and exercise instruction
- Conduct comprehensive followup visits for home health patients
- Guide patients in proper use of assistive devices and prostheses
- Maintain detailed patient documentation in compliance with healthcare regulations
- Collaborate with interdisciplinary healthcare teams to ensure optimal patient outcomes
- Monitor and evaluate patient progress adjusting treatment approaches as needed
- Promote wellness and provide patient education for longterm health maintenance
Qualifications & Requirements:
- Masters degree in Physical Therapy required
- Current state Physical Therapist license
- Minimum 1 year of home health care experience
- Valid drivers license and reliable transportation
- Current CPR certification
- Strong interpersonal and communication skills
- Proven experience with orthopedic and neurological rehabilitation
