Summary: Enroll coordinate services/ referrals and provide comprehensive nursing care via inperson and/or virtual visits to a caseload. Monitoring client vitals recording changes in conditions taking patient histories and organizing health records. Visits may include nursing assessments education guidance and support to families in their homes locations of their choice or via telehealth.
Duties and Responsibilities:
- Assessing patients: Home visiting nurses evaluate patients wellbeing.
- Maintaining records: Homevisiting nurses maintain accurate records of the services they provide.
- Connecting with care providers: Homevisiting nurses collaborate with other providers and physicians.
- Monitoring vitals: Home visiting nurses monitor and record patients vital signs when necessary.
- Developing care plans: Homevisiting nurses help develop and maintain patient care plans.
- Documenting visits and submitting information within the specified time frame.
- Performs other related duties as assigned.
Skills/Licenses/Certifications Necessary:
- Registered Nurse ADN/ASN
- BLS current certification
- Excellent verbal and written communication skills.

Education/Experience:
- Bachelor of Science in Nursing Registered Nurse who will complete BSN within one year
- Experience in Maternal Child Health Pediatric Health Mental Health and/or case management is a plus.
Physical Requirements/Working Conditions:
- Must be able to drive around county to complete visits
- Must have private area to contact clients and discuss protected health information
- Must be able to lift up to 15 pounds at times.
