Shift Schedule:
- fulltime and parttime position

We are seeking a qualified person to be part of our interdisciplinary team that provides social service case management to shortterm and longterm residents.
Working as a resident and family social service assistant you will be responsible for:
- advocating for resident rights
- evaluating and meeting resident psychosocial needs
- arranging for services such as hospice
- developing a comprehensive assessment and residentbased care plans
- exercising independent judgment making sound decisions and exhibiting flexibility when needed
- ensuring and providing therapeutic interventions to assist residents in coping with their transition and adjustment to their placement
- serving as liaison between residents families the facility and outside agencies
- providing oversight for our longterm care residents psycho/social needs
Required Qualifications
- bachelors degree in social work or in a human services field including but not limited to: gerontology sociology rehabilitation counseling psychology.
- excellent communication and listening skills
- ability to organize and prioritize effectively
- good problem solving and assertiveness skills
