Job Description

Hospice Social Worker

Location: Corinth MS
Job Type: Hourly Non-Exempt

About Us

At Legacy Hospice were committed to caring for patients and families with compassion and dignity. We are seeking a skilled and empathetic Social Worker (SW) to join our this vital role youll help patients and families cope with the emotional social and practical challenges that come with a terminal diagnosisbringing comfort and clarity during their most vulnerable moments.

What Youll Do

  • Conduct psychosocial assessments of patients and caregivers
  • Provide counseling to patients and families regarding emotional stress coping grief and advance care planning
  • Assist with advance directives funeral arrangements and navigating legal/financial decisions
  • Identify and coordinate with community resources agencies and support organizations
  • Educate staff and families about cultural and psychosocial needs
  • Document patient conditions interventions and psychosocial support provided
  • Serve as a liaison between families and care teams to ensure coordinated holistic care
  • Participate in interdisciplinary team meetings to develop individualized care plans
  • Provide crisis intervention and support for safety concerns (neglect abuse self-harm)
  • Participate in quality improvement initiatives and continuing education
  • Participate in on-call support as needed

Qualifications

  • Bachelors degree in Social Work (BSW) required
  • Masters degree (MSW) or LCSW strongly preferred
  • At least 1 year of experience in social work preferably in hospice home health or a community setting
  • Knowledge of end-of-life care grief support and cultural sensitivity
  • Strong written and verbal communication skills
  • Self-motivated organized and able to work independently
  • Current Basic Life Support (CPR) certification
  • Valid drivers license auto insurance and reliable vehicle required
  • Please note: Legacy Hospice is a drug-free workplace. Pre-employment drug screening is required including testing for marijuana.
