About the Position

The expected salary range for this role upon hire is $50197$65271.
This is a term position that ends on July 1st 2025.

WHO WE ARE LOOKING FOR


Join the Babies Born Healthy Program as aTermCommunity Health Worker and make a meaningful impact in our community! We are looking for passionate individuals with a minimum of 2 years of experience in a public health setting such as a health center clinic or in programs dedicated to Maternal Child Health Care and community health education.


This role requiresexpertisein delivering home visits and communitybased services. If you have exceptional customer service skills and a talent for building strong supportive relationships we want you toapply.


What Youll Be Doing

TheTermCommunity Health WorkerTermCommunity Services Aide (CSA) positionis responsible forthe following tasks:


  • Providing care coordination services to pregnant and parenting families under the Babies Born Healthy Grant.

  • Linking pregnant women to essential medical and social services.

  • Conducting home visits tofacilitateclients access to care and ensure theappropriate useof health resources.

  • Ensuring that all pregnant patients receive prenatal care.

  • Offering education on prenatal care and newborn care.

  • Ensuring that both mothers and newborns have a designated medical home.

  • Demonstrating knowledge of Department of Health and Human Services (DHHS) County and community resources and makingappropriate referralsto both internal and external programs to meet clients needs.

  • Teaching strategies to reduce barriers to medical care.

  • Conducting community outreach activities to promote the Babies Born Healthy Program and educate the community about safe pregnancy and parenting practices.

  • Collecting data for the Babies Born Healthy Program.

  • Participating in the Babies Born Healthy interdisciplinary team.

  • Provide Childcare during client prenatal appointments and during program activities

  • Oversee transportation rides for clients prenatal appointments and during program activities.

  • Utilizing Community Health Worker skills to reach clients in highrisk areas.


Minimum Qualifications

Experience: One 1 year of experience providing delivery of social health and/or publicassistance

services.


Education: Completion of high school or High School Certificate of completion recognized in the State

of Maryland.

Equivalency: An equivalent combination of education and experience may be substituted.


Special Minimum Qualification:

Maybe requiredto have bilingual ability (English and a foreign language) depending on position

assignment.


LICENSE:

Possession and maintenance at all times of a valid Class C (or equivalent) drivers license from the

applicants state of residence whenfor jobrelated duties.

  • Experience in a Public/Comm Health supporting pregnant and postpartum families and providing homevisits.

  • What strategies do you use to ensure that patients understand their treatment plan and follow through with it

  • Describe your approach to building relationships and trust with community members.

  • What strategies do you use to motivate people to make healthy lifestyle changes

  • What is your experience providing childcare
