Job Description

Hospice Nurse Practitioner (NP)

The Job
The Nurse Practitioners primary functions are to assist in overseeing the clinical care and treatment of patients and families to consult with the patients attending physician in consultation with the hospice physician and to provide clinical direction to the interdisciplinary group/team to ensure quality care.

Primary responsibilities will be:
  • Collaborate with the IDG/T in assuring appropriate evaluation and palliative treatment of terminal disease.
  • Consult with and assist in the IDG/Ts development of patient/family plan of care to assure quality care is provided.
  • Consult with and educate the patient/family regarding disease process selfcare techniques endoflife care.
  • Provide training to other staff as needed.
  • Perform and document visits to patient/family based on plan of care and patient/family need.
  • Interact with attending and consulting physicians as necessary regarding palliative care in collaboration with hospice physician.
  • Consult with and provides education to other IDG/T members on a prn basis for clinical issues relating to palliation.
  • Actively participate in the IDG/T. Communicate effectively timely and appropriately at all times.
  • Inform physicians and other personnel of changes in the patients needs and outcomes of intervention while evaluating patient/family response to care.
  • Complete facetoface encounters with patients as assigned prior to 3rd benefit period and each period afterwards.
  • Provide clinical assessment results to hospice physician after making facetoface encounters with patients as assigned.
  • Always use standard precautions per hospice policy.
  • Adhere to hospice policies regarding protected health information.
  • Keep up with and maintain education/knowledge in pharmacology and palliative medicine.
  • Provide oncall services to patients/families as assigned
Qualifications:
  • Education: Graduate from a Baccalaureate School of Nursing a Nurse Practitioner (NP) focused Masters or Doctoral Nursing Program
  • Licensure: Current state license as a Registered Nurse and as an Advanced Practice Nurse.
  • Current drivers license.
  • Two years experience as an Advance Practice Nurse clinical care setting preferred; hospice or home health preferred.
  • Nursing skills as defined as generally accepted standards of practice. Comprehensive knowledge of pharmacology palliative care and endoflife. Good interpersonal skills.
  • Reliable transportation and valid auto liability insurance.
  • Works in patients homes in various conditions; possible exposure to blood bodily fluids and infectious diseases; ability to work a flexible schedule; ability to travel locally; some exposure to unpleasant weather; PRN emergency calls.
  • Prolonged standing and walking required with ability to lift up to 50 lbs and move patients. Requires working under some stressful conditions to meet deadlines and patient needs and to make quick decisions and resource acquisition; meet patient and family individualized psychosocial needs.
