Within the context of a dynamic client and familycentred care model and in alignment with the organizations mission vision values and objectives the Social Worker Assistant:
Works under the general operational supervision of the Social Work Site Leader.
Receives daytoday work direction from Social Workers in a teambased environment.
Provides social work support services during the admission and discharge processes of patients and clients.
Assists clients families and facilities in accessing external and community resources.
Offers practical support related to benefits housing transportation finances and other community services.
Escorts patients to community appointments relevant to their admission or discharge planning.
Maintains a current supply of information and applications on available services agencies and resources.
Education
Completion of a recognized Social Service Worker or Social Service Diploma Program
or
An equivalent combination of education training and experience.
Skills and Abilities
Effective verbal and written communication skills.
Ability to interact professionally and respectfully with others.
Physical ability to carry out the duties of the position.
Strong organizational and prioritization skills.
Ability to operate related equipment as required for the role.

Your Day to Day
Collaborates with Social Workers to follow up on delegated tasks identified in psychosocial assessments.
Works closely with designated Social Worker(s) to provide followup psychosocial support during the admission and discharge process by:
Sharing relevant resource information
Assisting with completion of Release of Information and application forms
Supporting transportation arrangements when requested
Assists patients/clients in applying for external resources related to housing income transportation and community supports by:
Providing contact details brochures and application forms
Ensuring applications are completed and submitted properly
Provides practical support by:
Helping clients complete forms and write letters
Obtaining consent for the release of information
Arranging transportation and mailing benefit applications
