We are the community behavioral health organization responsible for the delivery of mental health substance abuse treatment coordination and developmental disabilities services for Kent County. We need individuals who are passionate about making a difference by helping others.
We are seekinga Case Manager who will be responsible for PersonCentered Planning to develop an Individual Plan of Service. Provide ongoing linking coordinating and monitoring of services.
This position is fulltime Monday through Friday from 8am to 5 pm; noon call or holiday requirements. This is an inoffice position at 790 Fuller Ave NE Grand Rapids MI 49503.

MINIMUM QUALIFICATIONS:
- Bachelors degree in human services related field (for example Social Work Education Psychology Sociology).
- Six months of fulltime experience working with persons with mental illness substance abuse disorder or intellectual or developmental disabilities (fieldwork and/or internship experience can apply)
- Possession of a valid drivers license and access to reliable transportation for job related use
- Strong organizational skills with experience in personcentered planning
- Demonstrated understanding of persons with severe mental illness and substance use disorders.
- Demonstrated communications skills that effectively convey information
- Demonstrated active listening i.e. listening to what other people say and asking appropriate questions
- Demonstrated time management skills
PREFERRED QUALIFICATIONS:
- Licensed BSW
- Knowledge of traumainformed theories principles and practices (includes multifaceted understanding of concepts such as community trauma intergenerational and historical trauma parallel processes and universal precautions)
- Lived experiences with mental illness/developmental disabilities/substance use disorders valued
