Were hiring a Hospice Medical Social Worker to join our growing team serving patients in Tucson AZ!
OVERVIEW
The Social Worker provides medical social services under the direction of a physician and Interdisciplinary Group to assist the understanding of significant social and emotional factors related to the patients health status and in development of coping mechanisms. They will collaborate with the patient and their support system(s) to develop and implement evidencedinformed personcentered care plans across the continuum of care for Home Health and Hospice patients and the interdisciplinary team of the Agency.
JOB SUMMARY:
- Assists the patient caregiver(s) physician and Agency care team to understand significant personal emotional environmental and social factors related to the patients health status on an as needed basis;
- Alternates between the two care settings (Home Health & Hospice) effectively and appropriately based on the care needed/provided;
- Contributes as a care team member to the development of comprehensive integrated plan of care for patients on a daily basis;
- Instructs care team members on community resources available to assist patients;
- Collaborates with hospitals and community Agencies to promote continuity of care;
- Assesses social and emotional factors related to patients illness(es) to determine ability to cope with daily living problems;
- Assists the patient and caregiver(s) to understand accept and follow medical recommendations;
- Assists the patient and caregiver(s) in utilizing community resources;
- Identifies gaps in community resources and stimulates resource development and/or improvement as needs arise;
- Visits patients according to Plan of Care completes a clinical note for each visit timely and accurately;
- Able to function as Bereavement Coordinator and supervise the provision of bereavement services reflective of patient/family needs if necessary;
- Other duties as assigned.

EDUCATION AND EXPERIENCE REQUIREMENTS
- Current license as aSocial Worker / MSW or BSW required
- Hospice and/or Home Healthexperience as aSocial Workerpreferred
- Current BCLS certification
- Current CPR card
- Current health certificate/physical examination and TB testing
- Valid Drivers license and reliable automobile
- Proof of current automobile insurance (MVR checked)
- Criminal background check and national sex offender clearance.
- Must have proof of up to date COVID vaccine or submit an exemption request to be reviewed/approved prior to hire
AverageHourly Pay Range
- $35 $37
