Job Description
Provides continuum of care for patients from admission to discharge. Assesses social emotional and financial needs of the patient. Assists in developing discharge plans with patients and their families. This includes referrals to community resources and/or placement in an appropriate facility following discharge. Assists with coordination of the discharge process to ensure a successful transition in collaboration with the health care team to achieve targeted patient outcomes. This role completes an assessment on the patient and caregivers throughout the hospitalization and outpatient followup as needed. This position identifies risk factors that the patient may experience after discharge and applies discharge interventions to lower the patient risk of an undesirable event post discharge. The Social Worker (MSW) bridges the discharge to outpatient provider with the patient and caregiver to address issues and questions.

Education:
- Bachelor or Masters degree from a recognized school of Social Work accredited by the Council on Social Work Education
Experience:
- Minimum one year of job related experience preferred in an acute care setting
- Knowledge of Medicare/Medicaid regulations
- Knowledge of community resources.
Licensures/Certifications:
- Current South Carolina License.
****Must have a License for consideration (out ofstate applicants will have 3 months to convert to SC)****

MUSC Health Marion Medical Center is a 124bed acute care facility that provides complete inpatient outpatient diagnostic medical surgical and emergency care. Marion Medical Center is Joint Commission accredited and included Mullins Nursing Center a 92bed longterm care facility. We believe in the power of people to create great care. MUSC Health Black River Medical Center supports a patientand familycentered approach to care. Family members are not considered visitors to a loved one who is hospitalized. They are key participants in support decisionmaking and care. We do not have visiting hour restrictions and two people are always welcome at the bedside 24 hours a day.
