Job Description

Job Path supports people with developmental disabilities as they make choices about their lives and play valued roles in their communities. Job Path encourages people to explore what they want out of life and to chart their own journeys. Whether its finding a job making a contribution in a volunteer role establishing a home or being part of the community the goal is to assist the person to lead a full and active life.

Job Path is seeking a Disability Resource Specialist whose primary focus will be to transition young adults from school based support into Job Paths Community Connections program which is a onetoone community based day habilitation program that is designed for adults to explore their interests develop skills and form meaningful relationships and connections.

The ideal candidate for this role will be able to think and act strategically and creatively. They will have strong interpersonal skills in order to recruit people who would thrive in Job Paths programs and tight organizational skills to keep track of billing requirements. Follow through skills are essential in order to ensure that people are efficiently able to begin receiving services.

Responsibilities include but are not limited to:
Recruitment and Intake into Community Connections
  • Attend fairs and meet transitional coordinators at various schools to identify prospective students who would thrive in Job Paths Community Connections program via a City Council transitional grant
  • Meet with participants and families to define the expectations of the program and describe its holistic approach and work with the Youth Transition Manager to determine who will be strong candidates for the program
  • Work closely with the Care Manager in obtaining OPWDD eligibility
  • Work with Intake Coordinator to ensure that participants have proper documentation to start receiving services
  • Upload documentation to CHOICES
Discovery and Activity Planning
  • Engaging in personcentered discovery and planning with people receiving support on the caseload which includes interviewing families and others who know them well to pinpoint interests skills needs and abilities
  • Work with family and community members to develop resources in the community including volunteer work internships social recreational and artistic activities.
Developing staff and providing support
  • Provide direct supports at least three days a week with the intention of connecting individuals to their communities via volunteer jobs classes social events and other community activities
  • Recruit and train direct support professionals and train them to effectively support individuals following Job Paths Code of Conduct
  • Provide assistance to individuals to join online events/support/groups and/or classes.
  • Encourage individuals to make choices and decisions in all areas of their lives and provide the opportunities to learn how to make decisions.
  • Support people in social situations to learn to feel comfortable and confident with others through appropriate interactions and build positive relationships
Administrative Duties
  • Generate casenotes to effectively bill to City Council grant for services provided while candidates are transitioning into receiving OPWDD day habilitation programing
  • Provide documentation of all other services rendered and ensure all regulatory requirements are met.
  • Tracking the required number services required in order to meet the requirements of the grant
Reports to: Senior Director of Community Connections for supervision and oversight of objectives and goals. The Director of Employment Intensive Services and Youth Transition Manager will also check in on a monthly basis to ensure that the overall number of service goals for the grant are being met.

Qualifications: Bachelors degree preferred or high school diploma with equivalent work experience and at least one year of experience working with individuals with special needs or in job development.

Salary: $40300 annual salary nonexempt from overtime
