Job Description

The Health Related Social Needs (HRSN) Community Engagement Coordinator will develop relationships with volunteers staff community partners government officials and communitybased health organizations to connect individuals with resources provide services to vulnerable populations and identify unmet needs in the community. Attends community meetings legislative meetings and calls and hosts community partners and develops solutions in coordination with Agency management.

Candidates must submit applications by 4/30/2025 in order to be considered for first round interviews.

MINIMUM QUALIFICATIONS The requirements listed below are representative of the knowledge skill and/or ability required.

EDUCATION and/or EXPERIENCE

  • High school Diploma or GED.

  • Two years of social services experience preferably direct service delivery with a vulnerable client population.

  • An equivalent combination of education and experience may be accepted.

CERTIFICATES LICENSES REGISTRATIONS

  • Driver qualifications required for this position. Valid driver license insured vehicle available for use on the job and a satisfactory drive record for the past 3 years are required.

  • Basic Life Support/First Aid Certification is required within first 30 days of hire.

  • Candidate must pass a comprehensive MWVCAA background screening prior to employment.

  • Candidate must pass preemployment and random drug/alcohol screenings.

KNOWLEDGE SKILLS AND ABILITIES

  • Basic proficiencies in computers and MS Office products database software and web tools. Must possess excellent planning organization and time management skills.

  • Ability to follow verbal and written instructions communicate effectively both verbally and in writing; write reports and business correspondence.

  • Ability to read analyze and interpret general business periodicals professional journals technical procedures or governmental regulations.

  • Bilingual EnglishSpanish skills are required for this position.

    • To qualify as bilingual staff must pass a bilingual exam demonstrating fluent speaking reading and writing in English and Spanish. The posted rate is inclusive of a 2 bilingual differential.

**ESSENTIAL DUTIES AND RESPONSIBILITIES: This job description is not intended to be allinclusive. Employee will also perform other reasonably related business duties as assigned by immediate supervisor and other management as required.

Relationship Management

  • Identifies develops and maintains key relationships to promote the HRSN program and its serviceswith essential community entities such as schools churches coalitions service providers communitybased organizations health care providers and other organizations.

  • Identifies and reaches populations with unmet needs for poverty services in line with MWVCAAs mission and values. Establishes connections with staff in critical roles for facilitating referrals to MWVCAA for services.

  • Cultivates relationships across diverse communities to connect the community and partner agencies to Community Action programs initiatives and services.

Engagement Activities and Events

  • Attends regular community meetings to foster relationship building collaboration and promotes equitable services for people in poverty. This includes: strengthening relationships with partners and identifying and addressing improvements to working together to authentically support those poverty

  • Provides an active reliable presence at community events and other venues and provides accurate information and referrals for community members seeking MWVCAA services. Promotes the Agency and antipoverty work best practices and advocates for the needs of our clients. Represents MWVCAA at various speaking engagements.

Reports

  • Maintain logs and issues monthly reports on community engagement and outreach activities to inform colleagues managers directors and senior leaders across the organization of community engagement progress.

  • Helps inform MWVCAA on successful and replicable community engagement practices as well as barriers to overcome.

SUPERVISORY RESPONSIBILITIES

There are no supervisory duties in this position.

  • Occasionally lift up to 50 pounds.

  • Manual dexterity for handling office equipment documents and phone.

  • Walking for several hours during various weather conditions.

  • Frequent driving in the community and between sites required.

WORK ENVIRONMENT

  • Indoor office work environment with frequent interruptions and demands.

  • Working with coworkers/clients over the phone via video conferencing and inperson.

  • Outdoor community outreach work during all types of weather on the streets and in homeless camps.

  • Community partner education health provider outreach in a variety of settings.

  • Occasional noise and distractions in work spaces. May experience loud agitated unpredictable behaviors from clients.

  • Occasional hazards associated with trash outdoor grass/brush biohazards and malodorous air.
