SALARY: $113000 to $178000

IDEAL CANDIDATE:
- Holds a Masters Degree in Social Work or a Bachelors Degree in Nursing
- 3 years of recent experience working as a RN or as a Social Worker in an Ambulatory or Acute Care / Hospital setting
- 1 year of recent experience in leadership managing people directly and overall operations of running a department in a healthcare setting
JOB DESCRIPTION
- Manager of Care Management (RN or MSW) at this leading Medical Center in Fullerton CA.
- This is a fulltime position that will lead and manage the Care Management Team of RNs and Social Workers.
- Under the direction of the Director Care Management the Manager Care Management is responsible for the overall daily operations and clinical practice of the assigned service areas. This position will oversee Case Managers and Care Management Coordinator support staff to assure the proper case management of our patients through the continuum of care.
- The Manager Care Management ensure the Case Managers to advocate for and assist the patient in the achievement of optimal health access to care and appropriately utilizing resources.
REQUIRED QUALIFICATIONS:
- Bachelors degree in nursing or masters degree in social work.
- 3 years of Experience in an ambulatory or acute care setting.
- Strong project management and operations experience.
PREFERRED QUALIFICATIONS:
- Masters degree in nursing or health administration.
- Case Management Certification (CCM or ACM) upon hire
- 3 years of Management experience in a Healthcare setting.
