The care and support we provide is highly valued by the people we care for and their loved ones but at present we are only reaching around 10 of dying people at the end of now one in four people in the UK with a terminal illness do not get the care or support they deserve at the end of their lives.
We are recruiting for a Healthcare Assistant to join our STARS (Supportive and End of Life Care Service) in Liverpool NorthWest.
Hours: Fulltime 37.5 hours per week (days)
Contract: 12months fixedterm
Base: Liverpool Hospice and the community
In this role you will work alongside a multidisciplinary team to provide assessment care planning review and evaluation of holistic care management plans individual to each patient.
As this is a communitybased role you will be responsible for planning your own work responding to changes in patient conditions prioritising care needs (including hygiene nutrition and hydration comfort monitoring vitals and more) as detailed in the personalised care plan and ensuring you escalate appropriately to senior staff within the multidisciplinary team in a timely manner.
