Nurse Practitioner/Primary Care Provider
Salary Range: $181749 $254499
Position Overview
As a Nurse Practitioner youll play a key role in delivering compassionate comprehensive care to our senior participants. Youll perform inperson assessments collaborate with our interdisciplinary team (IDT) and help shape highquality coordinated care plans. Key Responsibilities

Conduct inperson comprehensive health assessmentsincluding initial semiannual unscheduled and ongoing evaluations

Partner with physicians and the IDT to develop and adjust individualized care plans
-
Participate in discharge planning from both acute and longterm care settings
-
Provide continued care for participants residing in nursing homes through regular needsbased visits
-
Coordinate closely with internal and communitybased providers to ensure seamless effective care
-
Support quality improvement efforts by reviewing outcomes hospitalization data and specialist utilization at the direction of the Medical Director

Participate in a rotating oncall schedule for nights and weekends
Qualifications
-
Graduate of an accredited nursing program
-
Certified Nurse Practitioner with active Prescriptive Authority

Licensed as a Nurse Practitioner in the state

Minimum of two years experience working with geriatric or senior populations
-
Strong understanding of chronic care and geriatric best practices
-
Valid state drivers license and a clean DMV record
-
Proof of current auto insurance
