About PACE:
Philanthropists Gary and Mary West founded West Health with the mission of enabling seniors to successfully age in place with access to highquality affordable health and support services that preserve and protect their dignity quality of life and independence. The Gary and Mary West PACE (Program of Allinclusive Care for the Elderly) affiliated with West Health is a new nonprofit that delivers medical social and behavioral services to the frail elderly to the North County of San Diego. This PACE center will be entrepreneurial in nature and will focus on innovations and bringing new processes to the traditional PACE environment.

Position Summary:
Under the leadership of the Medical Director the Behavioral Health Coordinator will support both the PACE team and psychiatric and counseling partners coordinate patient care and work directly with patients to ensure consistent engagement with their care plans. As a member of an interdisciplinary care team you will collaborate with primary care providers psychiatric providers and other mental health specialists to promote quality care and improve participant outcomes.
Essential Functions:
Participant Engagement: Closely monitor and engage with a cohort of participants that exhibit high BH needs. Meet with participants who miss appointments to develop tailored strategies to support their attendance and engagement as well improve compliance with their BH treatment plans. Weekly case consultations in tandem with the PACE provider and/or BH consultant to review progress.
Provider Support: Assist PACE providers and BH vendors and by organizing monthly BH huddles for highrisk participants and ensuring smooth communication documentation and scheduling for BH care provision to participants in PACE.
Care Coordination: Schedule and coordinate BH visits actively working to reduce noshow rates through personalized outreach plans. Collaborate with RNCMs when participant is in inpatient or skilled nursing facilities to ensure BH needs are being met in those settings.
Integrated Collaborative Care Model for BH development and implementation: Conduct staff and BH partner interviews to identify pain points or barriers in BH delivery or access for PACE participants and propose solutions and develop timeline and resource identification for implementation under the guidance of PACE clinical leadership.
Resource Navigation: Serve as a liaison between participants and PACE social workers and providers offering guidance on local and state behavioral health medical and social support programs developing an internal resource database.
Documentation & BH status monitoring: Maintain accurate registry of high risk BH cases as well as all PACE participants with BH diagnoses with predetermined monitoring criteria and status.
Attend and participate in staff meetings inservices projects and committees as assigned.
Adhere to and support the centers policies practices and procedures.
Accept assigned duties in a cooperative manner and perform all other related duties as assigned.
Be flexible in schedule of hours worked.
May require use of personal vehicle.
Education and Certification:
High school diploma/GED required.
BA/BS in a related field (e.g. social work psychology)
Masters degree preferred.
Willing to take the certification course for Behavioral Health Coordinator (within 6 months of hire).
Trained in ProblemSolving Therapy as well as Motivational Interviewing or wiling to receive training.
CPR and BLS certification required.
Original valid driver license with proof of auto insurance.
Skills & Experience:
Minimum of two 2 years of demonstrated successful experience in psychiatric patient care coordination or working with behavioral health patients
Minimum of one 1 year of documented experience working with a frail or elderly population.
Working knowledge of local and state behavioral health medical and social resources.
Strong interpersonal and communication skills to build trust with participants and collaborate effectively with care teams.
Ability to work with ethnically diverse populations.
Ability to handle sensitive patient information with confidentiality and professionalism.
Effective oral and written communication skills.
Fluent in English and Spanish.
Medical Clearance:
Employees must have medical clearance for communicable diseases and uptodate immunizations before having direct participant contact.
