Using a Housing First approach Avalons Support Coordinators provide outreach intensive case management and housing support services to Avalon tenants and clients. The individuals we serve face many challenges including mental illness substance use disorders and/or chronic medical conditions in addition to chronic homelessness. Our clients live in Avalon Housing units with private landlords or in public housing located within Washtenaw County.
Support Coordinators collaborate with community partners to coordinate delivery of a broad range of services that meet the mental physical psychosocial and housing needs of our clients either through direct service or connections to appropriate resources.

While performing the duties of this job the employee will be required to sit for long periods of time drive a vehicle with passengers communicate with other persons by talking and hearing required to lift and carry items weighing up to 25 pounds and to operate computer hardware systems.
A valid Michigan Chauffeurs license and reliable transportation is required. You will be required to transport clients in your personal vehicle.
