Job Description
Do you have a passion for helping others and going the extra mile Would you like the opportunity to be part of a dynamic team within a growing family owned business At 360 SHS we have an exciting parttimeopportunity for acompassionate mission driven Licensed Practical Nurse (LPN) or Registered Nurse (RN). As a nurse at 360 you will be an integral part of ensuring that we are delivering exceptional care to our clients and families while providing support to our amazing team of caregivers.

Responsibilities
- Perform visits with clients and caregivers in the home
- Adjust care plans following visits to reflect any changes
- Evaluate client needs and identify opportunities in which we can provide additional support
- Identify opportunities in which we can empower our caregivers through education and coaching
- Document visit details and follow up notes in a timely accurate and comprehensive manner
- Work closely with our Clinical Director to be a professional presence in the community
- Collaborate with other team members and our community partners
Qualifications
- 1year experience as an LPN or RN in a home care setting
- Current state license as an LPN or RN
- Excellent interpersonal skills
- Valid drivers license and reliable transportation
- Ability to work a flexible schedule and travel locally
