Job Description

Crisis Practitioner

$6000 Retention Bonus!!

The candidate selected for this position may be eligible for a retention bonus of up to $6000. An initial sum of $2500 will be paid after the completion of six months in this role with the balance disbursed after twelve months of service. To qualify for the full bonus the individual must maintain employment in this position for at least twelve months. The bonus is nontransferable to other roles.


Salary

This area has been designated as a Minnesota Health Professional Shortage Area (MHPSA). Employees who primarily work in an MHPSA will receive a 15 differential pay in addition to their base salary (which is already included in the salary range above).

Practitioner

Base Salary: $54549.47 $63162.54/annually

Base salary MHPSA differential: $62731.90 $72636.92

The expected compensation range for this position represents a good faith estimate for this position and the specific compensation offered to a candidate may vary based on factors including but not limited to the candidates relevant knowledge training skills work location experience and/or FTE.

About Us

This mission of Canvas Health is to bring hope healing and recovery to peoples lives.

Canvas Healths mental health and substance use disorder providers clinicians and staff bring their diverse lived experience to help inform and guide the care they provide to clients. Our integrated programs and services offer coordinated care to people of all ages in their homes the community and in our clinics. We strive for excellence in clinical care as we serve those living with mental illness substance use disorders crisis unstable housing and trauma. Canvas Health is a nonprofit Certified Community Behavioral Health Clinic (CCBHC) specializing in serving clients who are on Medicaid.


Position Summary

This position is responsible for answering phones to triage and determine the need for mobile crisis response providing diagnostic and crisis assessment crisis intervention and additional therapeutic services as indicated as a member of a mobile crisis response team. Services are provided inhome (Mobile Crisis Response) officebased and telehealth.

North Branch MN


Schedule

40 hours per week Monday through Friday typical hours are between the hours of 8am to 5pm or 7 pm


Minimum Qualifications

Practitioner

  • Possess a bachelors degree or higher in behavioral sciences.

AND

  • Completed 2000 or more hours of delivering supervised services to clients with mental illness substance use disorder or emotional disturbance

OR

  • Currently serving a federally recognized tribe and meets the definition of vendor of medical care.

Preferred Qualification

  • Experience working with and/or delivering services to diverse populations
  • Experience delivering either inhome or crisis services
  • Experience with Electronic Health Record systems
