The Nassau County PINS (Person in Need of Supervision) Diversion program is designed to divert youth who are at risk from PINS petitions that require court involvement. PINS Diversion services families with children up to the age of 18 in Nassau County. The PINS diversion program provides short term intensive inhome familycentered interventions with an overarching goal that will prevent out of home placements teach problem solving skills to the families and linkage to a communitybased system of care.
The overall focus of Case Planners will be to improve individual functioning and family dynamics to prevent out of home placement for the youth. Case Planners will work flexible schedules and spend most of their work hours in the field. It is anticipated and expected that these case managers will be meeting with families in the evening and on weekends. Each Case Planner will have a caseload of 1525.
To be a successful Case Planner you must be ready willing and able to do the following always:
- Function within Agencys policies and procedures as outlined in the Employee Manual.
- Function within the agency policy and procedures as outlined in the program guide and applicable codes of conduct.
- Provide support to the clients and familiesi.e. the knowledge skills and tools necessary for them to maintain the child in their home.
- Achieve successful outcomes within the guidelines outlined in NYS County and HFY policy and procedure (shared).
- Perform other resulting and necessary work to meet the needs and requirements of the clients the program Hope for Youth oversight agency and any other regulatory obligations.

How you will generally spend your time in this job:
- Maintaining regular contact with the youth and their families at least once per week and implementing the various parts of the program model (including evenings nights and weekends) for caseload (approximately 1520 throughout Nassau County utilizing your own vehicle.
- Providing case management services life coaching parent skill training and making all collateral contacts as necessary.
- Regularly reviewing the service plan with the youth and family.
- Completing the initial CANSNY and all subsequent CANSNY as well as attending school meetings and court appearances with the families as necessary and being responsible for discharge/transition planning.
- Documentation and related paperwork
- Supervision staff meetings treatment team meetings and conferences
- Training and staff development
- Perform other resulting and necessary work to meet the needs and requirements of the clients the program Hope for Youth oversight agency and any other regulatory obligations.
To qualify for this job you need to be or have the minimum:
- BSW (or Bachelors in a related field) plus 2 years related casework experience or MSW.
- Excellent written verbal and computer skills
- Flexible schedule to accommodate clients and program.
- Bilingual Spanish/English speaking preferred.
- Valid NYS Drivers License in good standing
- Physical Requirements: read/write reports and log entries walking/standing/sitting for prolonged periods bending/stooping kneeling/squatting climbing stairs reaching overhead repetitive wrist/finger movements ongoing access to reliable vehicle and driving own vehicle for work
- Subject to oversight agency and other required background checks
