Job Description

Requirements: Graduate of NLS school of nursing; BSN preferred; Current RN license in WI and IL or be able to obtain both; Valid DL and reliable transportation. Responsible for the provision of care for an assigned group of patients/family based on the nursing process. The RN Case Manager is responsible for for the coordination of services provided by the Interdisciplinary Team assuring that the patient and the family needs are met and case is provided consistent with federal and state regulations Beloit Health Regional Hospice policies and procedures and hospice standard of practice.
