Job Description


Title:Housing Specialist

Division:Systems of Care Safe Option Support Hub (S.O.S)

Reports To:Director of Operations Safe Option Support HUB



Purpose: The incumbent should be an initiativetaking consumer focused professional. He/She/They will operationalize and manage placements for subway and street homeless individuals at a single point of access referral system for Safe Option Support (SOS) network. The SOS network is a newly launched multidisciplinary response to support individuals experiencing street and subway homelessness across New York State. The program design calls for a point person that will manage external referrals and placements oversight and track successful transitioning and report to the Governors office on outcomes and trends. Under the supervision of the Director the Housing Specialist will play a pivotal role in supporting New Yorkers experiencing homelessness as they transition from the streets and subways to housing stability. The incumbent will support the referral and placement process facilitate connection to appropriate services conduct assessments present data that is tracked and coordinate resources. In addition the Housing Specialist will review determinations and establish relationships with community partners across the state.


Education Requirements

Bachelorsdegreein social workPublic HealthPublic Policy orequivalent certification OR at least 2 years of equivalent comparable housing or real estate experience working with economically and socially disadvantaged individuals and or families.

Essential Duties and Responsibilities

  • Underthe supervision ofthe Director(SOS) the HousingSpecialistisresponsiblefor identifying appropriate placement for participants conducting assessments to determine participants housing readiness/needs and collaborating with CBC SOS/HUB Care managers to identify safe permanent and affordable housing placements.
  • Serve as client liaison to housing agencies both public private and with member organizations including being the direct contact with all program participants at CBC/SOS HUBs.
  • Assistclientsin thecompletionofapplicationsfor housingandrentalsubsidy programs.
  • Coordinatewithmemberorganizationsparticipantescortstosubsidizedhousingand apartment visits lease signings realtors property owners and other appointments.
  • Liaisonbetween allprogramparticipantsandstaffat SOS/HUBSandprovide guidance as it relates to housing related issues.
  • Inspect/approve potential housing placements to ensure they are appropriate for outgoingprogramparticipantsensuringappropriateplacementincomefamilysize etc. are suitable for placement.
  • Advocatesonparticipantsbehalftoobtainand/ormaintainhousingsubsidies (Advantage NYC Section 8 FEPS etc.
  • Attendingstaffmeetingstrainingasrequired andperformingspecialprojectsandother duties as assigned and cooperating with other providers as needed on client cases.
  • Maintainstrackingformsforplacements; providesaccuratereportsinatimely manner.
  • Performs otherdutiesasrequested.

Qualifications

  • Two years of comparablehousingorrealestateexperience.
  • Knowledgeofstrengthsbasedcounselingcognitivebehavioraltreatmentand behavior modification in practice
  • Experience serving or developing systems who serve persons with chronic health conditions behavioral health conditions persons experiencing homelessness and persons with Medicaid.
  • Abilitytocollaboratewithallstakeholders: membersnetwork families andgovernment staff in a caring and respectful manner and with due understanding of and consideration for cultural differences.
  • Superiorcommunication and writingskills.
  • AdvanceKnowledgeofMicrosoftofficeapplicationsincludingExcelReliasand Electronic Health records.
  • Abilityto manage multipleprojectsand ask forhelpwhenneeded.
  • Abilitytoparticipate ona teamto accomplishtasksor function independentlywhen appropriate.
  • Strongcustomerserviceskills andtheabilitytoanalyzedatatoresolveproviderand insurance service issues.
  • Ability and willingness to travel within NYC occasionally with program participants.
