PART TIME Clinical Social Worker Full or Limited License

DaVita has an open position for a Social Worker who will be a vital member of each patients core care team. Life on dialysis can be a difficult transition for many patientsand youll be there to support and advocate for them. In this vital role you will help patients understand their rights and responsibilities and guide them in managing the physical mental emotional and financial demands of End Stage Renal Disease.

What Can You Expect
Part Time 24 hour a week ideally 3 days a week with a Monday Wednesday Friday schedule at typical Business Hours
Ability to manage a group of 60 to 85 patients within an incenter setting

What Will You Bring
Masters degree in Social Work (MSW) required with a specialization in clinical practice
Fully/Limited Licensed in Michigan as required by state regulations.
Ability to cover multiple clinics to reach fulltime hours.
Two years of experience as a social worker in a healthcare setting preferred
Demonstrated knowledge of government and private insurance programs
Basic computer skills in MS Word Excel PowerPoint and Outlook as well as functional proficiency with DaVitaspecific applications within 60 days
