Job Description

JOB SCOPE:

The Assistant Director is responsible for assisting the Program Director in all aspects of residential operations to ensure compliance with all regulatory agency requirements as well as PCMH policies and procedures. Major areas of responsibility include ensuring quality case management and care for all residents maintaining comprehensive case records providing leadership and supervision of direct care staff and responding to crises.

ESSENTIAL FUNCTIONS:

  • Ensures that residents follow appropriate psychiatric and medical plans; approves individual service plans as needed. Integrates services of the Health Care Coordinator with other program and case management services.
  • Maintains case records in accordance with regulatory requirements; reviews individual case records and signs off as needed; approves admission and discharge summaries.
  • Provides assistance to residents around all Residential Counselor activities as needed.
  • Provides ongoing supervision to Residential Counselor and Front Desk Counselors as needed.
  • Maintains a resident caseload as needed.
  • Conducts and participate in case conferences.
  • Assists in scheduling and planning recreational and social activities.
  • Facilitates groups in the residence.
  • Reports all untoward incidents and completes followup on incidents.
  • Prepare written correspondence and notifications on resident related issues.
  • Conducts internal audits of clinical case records and all other program documentation to ensure compliance with city state and federal regulations.
  • Monitors compliance with all applicable safety and health code requirements.
  • Maintains productive working relationships with provider groups and referral sources.
  • Monitors maintenance staff to ensure the residences overall cleanliness and upkeep.
  • Adheres and facilitates adherence of all staff to personnel and residential policies and procedures as outlined in the PCMH Personnel Policies and Procedures Manual and the Residential Policies & Procedures Manual.
  • Provides oncall coverage of the site as needed.
  • Assists in the collection and submission of rental payments and receipts.
  • Enforces terms and conditions outlined for residents in all Lease and Lease Riders.
  • Where applicable helps to ensure compliance with LIHTC regulatory requirements including working with residents to obtain required documentation.
  • Participates in special projects assigned by senior management.
  • Performs other duties consistent with the goals and objectives of the program as assigned by the Program Director Regional Director or Director of Residential Services.
Salary:$29.00 hourly with a Bachelors Degree $31.50 hourly with a Masters Degree.

  • NYS OMH Licensed Housing Regulations NYS OMH Supported Housing Guidelines and/or NYC DOHMH OPRE Standards.

EDUCATION AND EXPERIENCE:

  • Masters Degree in a Mental Health discipline or related field is preferred. Minimum of a Bachelors Degree in a Mental Health discipline or related field with 3 years related experience.
  • Computer literate with knowledge of Microsoft Office spreadsheets and publishing software.
