Job Description

Community Unplanned Care Service Lead

  • Hours: 37.5 per week (part time considered)
  • Location: Peace Hospice Watford
  • Salary: Rennie Grove Peace Band 7 46148 to 52809 per annum (pro rata)
  • Closing date: 2 May 2025 at 12 noon
  • Interview date: 16 May 2025

The purpose of the Community Unplanned Care Service Lead is to provide leadership and high quality specialist palliative practitioner care skills advice and support to patients carers and those important to the patient. This role will directly manage a team of band 6 nurses paramedics and an HCA.

You will therefore need to demonstrate your abilities in the following areas:

  • Actively participate in the development of the organisations planned andunplannedcommunity model of care.
  • Offer advice support and education to the primary health care team as part of aninterdisciplinary team.
  • The management of a specialist palliative virtual ward caseload across the RGP Watford 3 Rivers and Hertsmere locality and support other RGP virtual wards within Hertfordshire as required to maintain our service.
  • To work from an interdisciplinary approach to care.
  • To engage in the full range of activities consistent with the role of a servicelead.
  • Demonstrate specialist skills and expertise in palliative care the post holder will deliverhigh standards of personalised care.
  • Demonstrate the ability to prioritise and reprioritise essential in this role.
  • Improve patient outcomes and experience by providing physical and emotional support totheir patients coordinating their care services and informing and advising them on clinicaland practical issues associated with their illness.
  • Participate activities such as audits and case reviews to improve the Hospice @Home service
  • Act as a Rennie Grove Peace Hospice ambassador within the community actively
  • demonstrating our values and positively promoting the specialist care we provide.
  • As required work across Rennie Grove Peace Hospice Care geographical area to support safe and effective service delivery.
