Coordinates care planning and delivery with the physician patient and family/caregivers other healthcare team members and Agency staff to facilitate optimal patient outcomes. Educates patients their families caregivers and other staff as appropriate in appropriate medical social work modalities and interventions. Provides modifies or discontinues medical social work modalities and interventions based upon an ongoing assessment/reassessment of the patients clinical status and in accordance with physician orders.
Demonstrates the ability to deal with pressure to meet deadlines to be accurate and to handle constantly changing situations. Demonstrates the ability to deal with a variety of people deal with stressful situations and handle conflict.
Will perform all job duties or job tasks as assigned. Will follow and adhere to all requirements regulations and procedures of any licensing board or agency. Must comply with all Benefis Health Systems organization policies and procedures.
Education/License/Experience Requirements:
- Current state clinical social worker license
- Masters degree from an accredited school of social work
- At least one 1 year of social work experience in a healthcare setting
- Valid state drivers license with proof of current insurance
