Description

Come help us elevate the lives of our residents! We are looking for Senior Social Workers to complete initial assessments in our LongTerm Services and Supports (LTSS) service area of Health and Human Services. These Senior Social Workers will be members of an interdisciplinary team of Public Health Nurses and Social Workers.The LongTerm Services and Supports assessments provide referral and planning are personcentered strength and needs based and serve to inform recipient of options determine eligibility level of care and service needs. The team administers Medicaid waiver and state programs that in addition to serving seniors serve individuals with brain injury mental health complex medical developmental and physical disabilities.

Location and hours:
These positions are hybrid and will be performed both onsite at a variety of Hennepin County offices at community locations to meet with people supported by LTSS programs and remote as job duties require. Work hours are typically Monday through Friday 8 a.m. 4:30 p.m.While these positions are designated as hybrid based on current requirements hires must reside in or relocate to Minnesota or Wisconsin. New Employee Orientation (NEO) for all new benefitearning employees is inperson in downtown Minneapolis. Details about NEO will be provided upon hire.

In these positions you will:

  • Conduct personcentered comprehensive social health medical and psychiatric assessments.
  • Create personcentered support plans utilizing formal services community resources and informal supports making referrals as necessary.
  • Determine applicants risk of institutional care and their level of care according to state and federal criteria.
  • Assist the applicant in identifying relevant goals along with specific supports that will enhance a persons ability to meet those goals.
  • Open eligible applicants to waivers and Personal Care Assistant (PCA) services according to availability.
  • Document data and case activities in state and county computerbased programs.

Need to have:
  • One of the following:
    • Licensed as a Graduate Social Worker by the Minnesota Board of Social Work.
    • Bachelors degree in social work psychology sociology or human services and18 semester 27 quarter) credits of graduate work in the below areas and three years or more of fulltime supervised experience as a social worker.
    • Masters degree or higher in social work or closely related field with course work emphasis in areas such as social work methods human growth and behavior and principles of social welfare.
  • A valid drivers license and daily access to reliable transportation to travel to a variety of Hennepin County offices and community locations and to visit clients in residential settings.

Nice to have:
  • One year or more of experience providing home and communitybased services.
  • Interpersonal organizational computer and personcentered interviewing skills.
  • A broad knowledge base related to payer sources for home and communitybased services and knowledge of existing communitybased services.
  • Knowledge of Medicaid Waiver programs and Medical Assistance (MA) Home Care Services.

Candidates with fluency in another language(s) in addition to English are encouraged to apply. Bilingual candidates who pass a language assessment may qualify for additional bilingual pay and duties.
