Key Responsibilities

As a Behavioral Health Clinical Team Manager you will:

• Plan direct and oversee the delivery of home health services for behavioral health patients.
• Coordinate patient care including assignments referrals and communication with providers and families.
• Ensure patient care plans are continually assessed developed implemented and updated.
• Maintain compliance with regulatory guidelines ensuring timely initiation of care.
• Process and coordinate physician orders staff assignments and care team communication.
• Review and approve patient care plans and evaluate clinical appropriateness of changes.
• Integrate input from all involved physicians into a cohesive patientcentered plan of care.

Qualifications

• Education: Associates Degree in Nursing or related field (required).
• Experience:
  • 2 years as an RN in Home Health or Behavioral Health.
  • 1 year of supervisory experience.
• Licensure: Current Registered Nurse (RN) license in Insert State.
• Technical Skills:
  • Proficiency in Workday and HomeCare HomeBase.
  • Proficient with MS Office Suite (Word Excel Outlook).
