Job Summary:

The Referrals Manager plays an integral part in the care coordination of all DAP Health patients. The Referrals Manager ensures high quality and timely coordination of care for patients to reduce any delays and barriers to care for all DAP Health patients. In addition the Referrals Manager is responsible for providing necessary data for ongoing monitoring of Referral Utilization and processing. While maintaining patient satisfaction and safety the Referrals Manager works closely with all necessary departments to promote efficient and solidified processes throughout the organization.

Supervisory Responsibilities:

  • Review weekly timesheets for accuracy review employee submissions and updates as needed; approve timesheets by required timeline;
  • Conduct oneonone meetings (monthly at a minimum frequency preferably biweekly) scheduled conversations and annual performance reviews with direct reports by required timeline;
  • Attend mandatory Managers Learning Collaborative trainings;
  • Partner with People Operations to:
    • Develop a learning plan for employee development goals to provide tools needed to excel in their position;
    • Implement and follow a process to address performance issues as they arise;
    • Prepare a comprehensive onboarding process for each specific position for all new hires;
    • Address serious issues that may require interventions or investigations.
  • Develop and monitor departmental and program/project operating budgets costs and schedules;
  • Supervise lead coach and use best management practices to improve staff performance;
  • Support and model the identified vision values and behaviors of the organization.

Essential Duties/Responsibilities:

  • Develop implement and monitor a comprehensive training program for all new & existing Referrals staff. To include but not limited to: Visio workflows step by steps memos policies etc;
  • Participate in quality improvement (QI) processes for referrals department;
  • Active participant in Training & Implementation Committee (TIC);
  • Coordinate and provide clinic coverage as needed;
  • Provide ongoing updated and accurate referral data/reports. (i.e. backlog completion compliance utilization message response time etc.;
  • Monitor and maintain an ongoing quality assurance review of referrals being processed by DAP Health staff via measurable audits and reporting;
  • Remain informed of all current and new changes in health plan referrals processes;
  • Develop and implement policies and processes as they pertain to referrals workflow and improvement;
  • Maintain a high quality and patient focused referral program/dept;
  • Develop and maintain positive relationships with health plan representatives;
  • Develop and maintain positive collaborative relationships with specialty offices within DAP Health service areas;
  • Maintain an ongoing Provider educational curriculum as it pertains to referrals in collaboration with the Medical Staff Office;
  • Facilitate monthly Referrals meetings with all DAP Health clinic locations;
  • Maintain accurate Provider specialty directory in EPIC as well as the Referrals SharePoint site;
  • Maintain a robust informational Referrals SharePoint site for all Referrals staff;
  • Develop and maintain informational Referral brochures for patient education;
  • Seeks assistance of supervisor when unable to process all referrals to avoid backlog and the clinical consequences of delays;
  • Provides quality care to the patients of DAP Health within the scope of practice outlined by state or federal law;
  • Deliver care in accordance with established standards of care and accepted community standards;
  • Understand the organizations commitment to provide high quality patient care. Promote a Patient centered environment;
  • Other duties as assigned.

Qualifications

Required Skills/Abilities

  • Excellent oral and written communication skills;
  • Effective interpersonal skills;
  • Proficient in Word Excel PPT and Visio;
  • Proficient in use of i2i Tracks Referral Manager preferred;
  • Accuracy is essential close attention to detail neatness is a priority
  • Ability to:
    • Manage time well;
    • Use office equipment i.e. copier and fax machine;
    • Use a computer;
    • Multitask work independently accurately and with minimum supervision;
    • Do math organize and prioritize workload work effectively and efficiently under stress;
    • Supervise understand and follow instructions;
    • Proficiently read write speak and understand English;
  • Bilingual in Spanish/English preferred.

Education and Experience

  • Current BLS certification obtained through the American Heart Association or American Red Cross;
  • Valid drivers license and current personal auto insurance;
  • Five years medical field experience with understanding of insurance and medical terminology;
  • Minimum of two years of leadership experience;
  • Minimum of five years ofexperience in processing referrals in a primary care setting.

Working Conditions/Physical Requirements

  • Ability to lift/move up to 40 pounds move from place to place and stand for long periods of time;
  • Abilityto travel to DAP Health locations throughout San Diego and the Coachella Valley as required.
