About the TCP & Lead Tenant Program

Targeted Care Packages (TCP) enables children and young people from Residential Care and Home Based Care to transition into care arrangements where their needs will be placements could include with parents family or friends a foster carer independent or semiindependent living or other form of care.

The aim of Targeted Care Packages is first and foremost to achieve better outcomes for children and young people in outofhome that end the best interests of children and young people will guide all decisionmaking when considering care and support options to ensure an individually tailored response.

Lead Tenant provides accommodation and support to young people between the ages of 16 18 years who are requiring intensive support to develop independent living and life skills. The program provides young people the opportunity to live in an independent setting while being supported by live in volunteers. The program aims to assist these young people to enhance their independent living skills in preparation for their exit from care at age 18.

Role Opportunity

TCPS have the opportunity to positively impact on young people and their families in the local community.

You will engage with young people families and local communities towards building a sustainable and secure placement for the young person.

Successful TCPs will play a consistent role with the child and young persons best interest the role will provide the overarching case management aimed at minimising the protective concerns and maximising personal development in accordance with case planning and the childrens Court directions.

Liaise with relevant services and advocate on behalf of children & young people to ensure access to resources where necessary.

About You

Experience working with young people with the ability to achieve positive outcomes for young people through a strengthsbased outcomesfocussed case management approach.

To be able to create a positive and safe environment for children and young people you will need to be strong communicator with the capacity to advocate engage and negotiate with relevant stakeholders.

A minimum of a Diploma in Social Work Youth Work or Community Services is required for this role however those currently studying in the field will also be considered on an unqualified rate.
