Job Description

Summary:
As a Telephone Triage Nurse you will uphold our Mission Vision and Values. The Nurse will assess evaluate and prioritize urgency of medical needs through telephone communication with patients following established standards practices and protocols. This is a full time 40 hours a week position. After training this position will be hybrid remote. A typical schedule for this role is one day in office and four days remote working.

Responsibilities (How this position enhances our Mission Vision and Values)

Mission Improve the Quality of Life of Others:
  • Triages calls with the patient to determine the appropriate level of care necessary coordinates with care teams and schedules appointments accordingly.
  • Schedules appointments for new and returning for all health related provides excluding Family Planning and WIC.

Vision ODHS preferred partner in East Central Indiana for improving the wellbeing of individuals and the communities in which they live:
  • Observes records and reports to physician/nurse practitioner patients condition and reaction to and treatments as indicated.
  • Provides patient education relating to current conditions as appropriate.
  • Consults with Chief Medical Officer and/or Director of Clinical Operations regarding unresolved client/patient problems.

  • Implements the nursing process and communicates with the physician/nurse practitioner regarding concerns and/or problems regarding the client/patient.
  • Maintains and reviews patients records charts and other information.
  • Maintains flexibility nursing assignments to include but not limited to followup chart audits and on floor nurse when needed.
  • Develops and maintains ongoing knowledge of nursing policies and procedures and standing orders and implements as appropriate.
  • Participates in professional development activities and maintains professional affiliations.
  • Attends required meetings and inservices and participates on committees as requested.
  • Reports diagnostic results to the identified team member within an identified timeframe.
  • Maintains patient confidentiality.
  • Performs all other duties as assigned.

Education and Experience:
  • Minimum graduate of an accredited school of licensed practical nursing.
  • Prefer two years of professional nursing experience in clinic/office setting.
  • Prefer 12 years of telephone triage nurse responsibilities.
  • Possession of an Indiana State Nurse license.
  • CPR certification and TB certification required to be maintained through ODHS post hire.

  • Ability to react calmly in emergency situations.
  • Ability to read understand and follow detailed written and oral communication.
  • Skill in applying and modifying the principles methods and techniques of professional nursing to provide ongoing patient care.
  • Knowledge of organizational policies regulations and procedures to administer patient care and common safety hazards and precautions to establish a safe work environment.
  • Skill in establishing and maintaining effect working relationships with patients medical staff and the public.
  • Skill in initiative listening judgment discretion and decisionmaking within the guidelines of the policies procedures and standing orders of the Health Center.
  • Skill in preparing and maintaining records charting and writing reports.
  • Ability to prioritize work assignments work independently and perform duties efficiently.
  • Understanding of individuals socioeconomic status and lifestyle with respect for individual differences.
  • Supports ODHS outreach events.
  • Supports other departments as needed & applicable.

Physical and Environmental Requirements:
  • Frequent exposure to communicable diseases toxic substances medicinal preparations and other conditions common to clinic environment. Tasks involve contact with blood or body substances.
  • Requires standing walking or sitting for long periods of time. Occasional bending stretching or lifting up to 40 pounds.
  • Requires the ability to work under stressful situations.
  • Requires corrected vision and hearing to normal range or use of approved adaptive equipment.
