Were seeking a dedicated and empathetic Case Worker to join our Specialised Homelessness Services team. In this vital role youll work directly with individuals and families facing complex challenges helping them access the support resources and stability they need to build brighter futures. Your Contribution

The Case Worker is responsible for the provision of case planning and case management in support of client needs. This position also performs all duties required to ensure the operations of the service are in line with the programs duty of care practice framework established protocols and the service agreement.

Duties & Responsibilities

  • Manage incoming referrals in accordance with processes
  • Complete intake and assessment processes with the incoming client within a timely manner
  • Collect rent monies and report rental arrears and provide support with the actioning of rent arrears
  • Develop case plans collectively with clients relative to their needs and actively provide case management support to clients
  • Develop and maintain effective relationships with clients and provide guidance and support for clients while assisting in the development of their skills
  • Provide information support and advocacy for clients accessing the service
  • Ensure an active commitment to case coordination with other providers referenced in the case management plan this includes: regularly reviewing and updating plans and recording actions
  • Participate in case referral meetings/processes
  • Participate in inductions for residents
  • Participate in conducting Strength and Needs Assessments
  • Promote client engagement in special events and important dates
  • Develop and facilitate group activities and programs minimum once per week during the Case Workers shift evidenced by a structured weekly planner
  • Deliver structured information sessions on topics relevant to clients (e.g. financial relationships court hobbies phones cooking shopping culture etc.
  • Liaise with the Practice Lead in relation to all client related matters including client needs engagement issues observations or concerns
  • Maintain necessary client and operational information including logbooks and case notes daily
  • Ensure secure management of all program files and compliance with applicable privacy legislation and policies

Required Qualifications and Experience

  • Minimum Cert IV in Community Services Child Youth and Family Intervention or equivalent or Bachelor of Social Work or equivalent
  • Hope and Healing Certificate or willing to obtain
  • Experience operating within a case management model
  • Demonstrated strong capabilities around building relationships with and motivating vulnerable people assessing needs and advocating for the client
  • Demonstrated experience/positive engagement when working with persons of Aboriginal and/or Torres Strait Islander backgrounds and an understanding of cultural competency
  • Have an understanding of the historical social cultural and economic issues affecting Aboriginal and Torres Strait Islander communities and young people today
  • First Aid and CPR
