Open Door is seeking a fulltime Family Nurse Practitioner (FNP) or Physician Assistant (PA) for its medical site in Mount Kisco NY. The NP or PA will report to the site Medical Site Director.
Responsibilities
- Provide high quality primary care to pediatric and adult groups.
- Provide immediate patient care and where necessary refer the patient to a primary care team for more comprehensive care.
- Develop and implement a plan of care in collaboration with other members of the care team the patient family members and significant others.
- Ensure high quality patient care is delivered costeffectively and consistently.
- Provide appropriate specialist referrals followup and hospitalization when indicated.
- Maintain electronic medical records in compliance with Open Doors standards with high efficiency.
- Partner with Open Doors Behavioral Health team and resources.
- Maintain familiarity with ongoing research and development in relevant fields.
- Ensure performance improvement initiatives are supported with timely and accurate data and consistent actions.
- Represent Open Door in community outreach activities.
- If Designated as responsible for Bio Hazardous Waste removal complete online training and receive certificate prior to completing this task.
Requirements
- Degree and licensure to practice as a Nurse Practitioner or Physician Assistant in NY.
- Bilingual: English and Spanish.
- Minimum of 2 years relevant experience; experience working in emergency urgent care or walkin clinic desirable.
- Outstanding bedside manners: patientcentric with empathy excellent active listening and communication skills engaging and efficient.
- Outstanding clinical capabilities and judgment.
- Focused on collaborative teambased care.
- Responsive and capable of working in a high accountability environment.
- Comfort level with using an electronic health record.
- Flexible.
- Excellent judgment and ability to prioritize resources where they will be most useful.
- Comfortable receiving and providing constructive feedback.
- Engaged in learning and continuous quality improvement.
- Outstanding written and verbal communications skills with a culturally diverse patient and staff population.
- Passionate about providing high quality primary health care to lowincome underserved residents and those lacking access to health care.
