Title: Targeted Case Manager KY MOMS MATR
Worksite: Warren Service Center
County: Warren regional 10 counties
Hours Status: FullTime MF 8a5p
Pay Range: $15.87 hourly
Brief Job Description:
As the case management you will be advocating and assisting pregnant and up to 1year postpartum clients who have risk factors that might interfere with achieving a healthy pregnancy and infancy. Provide direct services to pregnant and postpartum women as well as working with communities to increase awareness of pregnancy risk factors and resource availability.

Qualifications:
- Bachelors degree in human services (Psychology Social Work Sociology Family studies Counseling Public Health Special Education Education.
- Oneyear postgraduation minimum experience working with SUD adults; or
one year of case management experience; or
6 months if possesses masters degree in approved field.
- Valid drivers license and current automobile insurance.
