Job Description

Summary Statement

This is the full performance level of work providing case management services to youth in residential settings. Work includes coordinating the youths treatment plan and monitoring progress and achievement of goals to prepare youth for release and transition back to their home community and school. Positions at this level typically report to a technical supervisor.

Essential Functions

Essential functions are fundamental core functions common to all positions in the class series and are not intended to be an exhaustive list of all job duties for any one position in the class. Since class specifications are descriptive and not restrictive incumbents can complete job duties of similar kind not specifically listed here.

  • Provides case management services to youth in a residential setting by coordinating and monitoring the delivery of services and treatment plans for youth committed to a secured facility.
  • Reviews case files including criminal and social history prepares screening and placement reports and records assesses youth needs and recommends appropriate rehabilitative programs and services while in the facility to encompass responsibility for discharge planning.
  • Conducts interviews with youth to explain terms and conditions of supervision commitment or detention and to explain behavioral modification goals processes and procedures.
  • Assists in the development modification or review of Treatment Service Plans as part of an interdisciplinary team; participates in treatment team review meetings that identify the youths needs treatment objectives and services behavior modification goals and release or aftercare planning.
  • Outlines youth strengths and areas that need to be addressed objectives methods to be used and anticipated results.
  • Obtains and coordinates services and resources needed to ensure rehabilitation plans and objectives are met.
  • Conducts group and individual behavioral modification counseling sessions for youth such as anger management and victim awareness and provides advice and assistance to youth concerning employment living conditions recreation lifeskills and other interests or concerns.
  • Assesses the youths progress towards rehabilitation and institutional adjustment; evaluates information and prepares reports describing program participation institutional adjustment and summary recommendations for or against discharge to aftercare.
  • Prepares and maintains social histories classification reports progress notes discharge summaries and chronological records.
  • Oversees an assigned group of youth during education recreation and work detail activities.
  • May act as a liaison to community organizations to develop and coordinate aftercare plans and services for youth.
  • May prepare and present testimony for court proceedings in the adjudication and disposition of juvenile cases.

Job Requirements

JOB REQUIREMENTS for Youth Rehabilitation Treatment Specialist
Applicants must have education training and/or experience demonstrating competence in each of the following areas:

  1. Possession of a Bachelors degree or higher in Behavioral or Social Science or related field.
  2. Knowledge of working with atrisk youth.
  3. Knowledge of case management which includes assessing planning developing implementing monitoring and evaluating options and services to meet an individuals human service needs.
  4. Knowledge of making recommendations as part of a clients service plan such as clinical treatment counseling or determining eligibility for health or human services/benefits.
  5. Knowledge of interviewing toobtain facts explore issues and identify courses of action.
  6. Knowledge of behavior management such as understanding explaining describing and predicting behavior studying the connection between behavior and its causes and identifying appropriate behavior modification strategies for individual situations.
  7. Six monthsexperience in interpreting laws rules regulations standards policies and procedures.
  8. Six months experience in narrative report writing.
  9. Possession of a valid Drivers License (not suspended revoked or cancelled or disqualified from driving).

Additional Posting Information

Incumbents working four or more hours of work between the hours of 6:00 p.m. and 8:00 a.m. the following day will receive 5 shift differential pay.

This position is a classification organized under an exclusive bargaining representative (labor organization) that has been elected by employees as their representative for collective bargaining and other applicableterms and conditions of employment in accordance with Title 29 Chapter 59 and Title 19 Chapter 13 and 16. This position is covered by a collective bargaining agreement based on its individual terms.

Preemployment Drug Testing: Upon a conditional offer you must undergo preemployment drug testing as part of the hiring process.

Criminal background check:A satisfactory criminal background check is required as a condition of hire. The recruiting agency may require the applicant to pay for the criminal background check as part of the conditional offer of hire.
