Job Summary
The University Counseling Center UCC at the University of Utah enthusiastically welcomes applications for a 12month fulltime 40 hours/week) Care Manager to become an integral member of our dynamic multidisciplinary team. Responsibilities include: provision of facetoface care management for students in need of longer term or more specialized treatment shortterm skillsbased counseling for students referred to the community community referral followup supervision of two social work practicum students who support the functions of the care manager collaboration and outreach with campus partners who provide case management for University of Utah students and active collaboration with community mental health providers and agencies including those who offer free and low cost treatment. Other relevant duties as assigned. Candidates must also demonstrate excellent human relations skills and effective verbal and written communication skills
Responsibilities
Provides case management for students requesting or are recommended for a transfer of care out of UCC services Provides case management for inhouse referrals to internal UCC services Provides follow up case management for clients of concern as needed Provides case management for students utilizing university contracted thirdparty telehealth services Processes requests for reassignments when established UCC clients have not been seen by assigned counselor in over 3 months or longer Facilitate weekly team meeting and participates on UCC Clinical Review Team to provide perspectives from case management services Partner and build relationships with campus and community partners to ease process of establishing care/services for students May provide supervision of undergraduate/graduate students in their provision of UCC case management tasks/services Rotate weekly consultation groups to meet with clinicians re: case management services and mutual clients May provide some individual counseling based on agency needs/demands for services This job description is not designed to contain or be interpreted as a comprehensive inventory of all duties responsibilities and qualifications required of employees assigned to the job. Minimum Qualifications
Current Licensed Clinical Social Worker (L.C.S.W. certificate for clinical practice and two years directly related experience or equivalency (one year of education can be substituted for two years of related work experience); demonstrated ability to work in a multidisciplinary team; ability to communicate verbally as well as in writing; and experience with conflict resolution when counseling individuals couples and groups required. Experience working with community resources instructing and supervising Social Workers Interns and other caseworkers preferred. Five years of experience may be required in some areas. An L.C.S.W. is earned after completing 4000 hours of Social Work experience and a passing score on the L.C.S.W. exam. This position is patientsensitive and must fulfill all associated requirements.
